Risk incidence Occurrence Solution

1) In reference to your proposed Database Design Proposal,
consider a situation where you want to track (and ultimately reduce) the occurrences
of a risk incident within the environment you selected.

2) Identify and select an incident in which you are
interested. An example of an incident could be that which is acquired or
occurred during hospital stays.

3) Develop a 750–1,000 word proposal from the perspective of
the Health Information Manager, which includes a specification of the
requirements, the proposed solution, the database design and/or modifications
to existing databases, and a breakdown of responsibilities among staff to collect,
analyze, and disseminate the information.

4) This assignment uses a grading rubric. Instructors will
be using the rubric to grade the assignment; therefore, students should review
the rubric prior to beginning the assignment to become familiar with the
assignment criteria and expectations for successful completion of the
assignment.

30.0 %Proposal Components: Requirements, Solution, and
Database Modifications

Description and assessment of the criteria are not outlined
or outlined poorly. Fails to formulate clear possibilities for change.

Minimally outlines the rationale for the design chosen and
lists limitations, and possible extensions. Ignores or superficially assesses
areas of difficulty or modification needs for the database. Formulates vague
and indecisive solutions.

Outlines the rationale for the solution, its requirements,
limitations, and possible extensions. A surface level assessment of the areas
of difficulty or modification of the database is offered. Expresses possible
solutions. Minimal detail is used.

Describes the rationale for the solution, its requirements,
limitations, and possible extensions Assessment is direct, competent, and
appropriate of the areas of difficulty or modifications needed for the
database. Formulates possible process changes or solutions. Some details are
present.

Describes in-depth the rationale for the solution, its
requirements, limitations, and possible extensions. Thoughtfully assesses and
evaluates areas of difficulty or modification for the database. Formulates
clear and precise process changes. Details and examples are present throughout.

10.0 %Analysis

Analysis of the criteria is not outlined or outlined poorly.

Ignores or superficially evaluates the incident and draws
unwarranted or fallacious conclusions. Lacks supportive evidence and details.

Surface level of evaluation of the incident is offered.
Minimal support for all components is presented.

Analysis is direct, competent, and appropriate for the
incident. Claims and ideas of the criteria are supported.

Analyzes and evaluates incident from a critical perspective
and draws warranted, judicious, nonfallacious conclusions. Information and
evidence are accurate and appropriate.

stated in the assignment criteria.

Content is incomplete or omits some requirements stated in
the assignment criteria.

Content is complete, but somewhat inaccurate and/or
irrelevant. Research is inadequate in relevance, quality, and/or timeliness.

Content is comprehensive and accurate, and rationales are
clearly stated. Research is adequate, timely, and relevant, and addresses all
of the issues stated in the assignment criteria.

Content is comprehensive, accurate, and persuasive;

10.0 %Research

No outside sources were used to support the assignment.

Few outside sources were used to support the assignment.
Limited research is apparent.

Research is adequate. Sources are standard in relevance,
quality of outside sources, and/or timeliness.

Research is timely and relevant, and addresses all of the
